Good question. I guess probably when a big media outlet notices the anal-only lifestyle movement, and does a report on it, and lots of people start speaking about it. Or else if some famous person declares him or herself to be anal-only, what would also result in media coverage and lots of people talking about it on the Internet. Search engines penalize old content, If lots of people start using a word in a new context at once, the new meaning would probably rise fast in the results.
