"Anal only" is indeed a slightly confusing term, but I think virtually everyone who practices it or talks about it uses the same interpretation as you, e.g. "no vaginal penetration", and that is always what I intend when using the term. I kind of see it as vaginal and anal being the two primary sex acts and so "anal only" is selecting within those, without excluding secondary sex acts/foreplay like oral, etc. A bit of a rationalization for the term, perhaps, but it works for me.
